29 CFR 1910.178(l)(6): The employer did not certify that each operator has been trained and evaluated as required by this paragraph (l): Worksite located at 912 Highway 30, Duncan NE and throughout the facility: The employer failed to ensure that a written certification for each fork truck operator was written and maintained as outlined in the standard. The certification shall include the name of the operator, the date of the training, the date of the evaluation, and the identity of the person(s) performing the training or evaluation. Abatement certification is required for this violation. The abatement certification sheet is enclosed with the citations.
