29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment were used to support scaffold platforms when they were not specifically designed by the manufacturer for such use: A) Job site Media Center building, on or about December 10, 2014, a steel personnel basket being used on a Pettibone forklift to lift an employee in the air was missing a section of the mid-rail of the guardrail system, exposing the employee to a fall hazard over 10 feet. B) Job site Media Center building, on or about December 10, 2014, a steel personnel basket being used on a Pettibone forklift to lift an employee in the air extended more than 10 inches beyond the wheel base, exposing the employee to a fall hazard over 10 feet. C) Job site Media Center building, on or about December 10, 2014, the employer did not have written permission from the manufacture to use a Pettibone forklift to lift an employee in the air in a personnel basket, exposing the employee to a fall hazard over 10 feet.
