29 CFR 1910.146(c)(2): The employer did not inform exposed employees, by posting danger signs or by any other equally effective means, of the existence and location of and the danger posed by the permit spaces:  a) Production department - The employer failed to post a sign reading "DANGER-PERMIT REQUIRED CONFINED SPACE, DO NOT ENTER" for the 3200 gallon tank containing Soy Based Adhesive Remover with an ingredient such as Diethyl Glycol and the 700 gallon tank containing Chemsafe Green D with an ingredient such as Butoxyethanol.

29 CFR 1910.1200(e)(1)(i): The employer's written hazard communication program did not include a list of the hazardous chemicals known to be present at the workplace.  a) Throughout facility - The employer failed to provide a list of chemicals present at the workplace when employees were working with Butoxyethanol, Diethyl Glycol Monobutyl Ether, Propane and Hydrocarbon mixture. Employees were exposed to skin, eye and respiratory tract irritations.
