At the facility located at 175 Heritage Drive, Pataskala, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2014, a description of work-related injuries and illnesses.  On or about August 12, 2014, an employee had a knee injury but there was no description of which knee was injured.   b. At the facility located at 175 Heritage Drive, Pataskala, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2014, a description of work-related injuries and illnesses.  On or about   September 3, 2014, an employee had an abdominal injury but there was no description of the specific injury.  c.  At the facility located at 175 Heritage Drive, Pataskala, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2014, a description of work-related injuries and illnesses.  On or about September 12, 2014, an employee was injured on a conveyor but there was no description of the specific body part injured.  d.  At the facility located at 175 Heritage Drive, Pataskala, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2014, a description of work-related injuries and illnesses.  On or about October 20, 2014, an employee injured an ankle but there was no description of which ankle was injured or of the specific injury.  e.  At the facility located at 175 Heritage Drive, Pataskala, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2014, a description of work-related injuries and illnesses.  On or about October 23, 2014, an employee was struck in the side by a cart, but there was no description of the specific injury or what the employee was doing when injured.  f.  At the facility located at 175 Heritage Drive, Pataskala, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2014, a description of work-related injuries and illnesses.  On or about November 11, 2014, an employee cut a finger but there is no description of what the finger was injured.  g.  At the facility located at 175 Heritage Drive, Pataskala, Ohio, at the time of the inspection, the employer had not recorded information in column (F) correctly for calendar year 2014, a description of work-related injuries and illnesses.  On or about November 19, 2014, an employee injured a middle finger but there is no description of which middle finger or of what the employee was doing when injured.  Speed Commerce, formerly Speed FC, Inc., was also previously cited for violations of this Occupational Health and Safety standard or its equivalent standard (29CFR 1904.29(b)(1)), which was contained in OSHA inspection number 314587312 , citation number 2, item number 1 a, citation number 2, item number 1 b.  The citations were affirmed as a final order on September 22, 2010, with respect to a workplace located at 107 Heritage Drive, Pataskala, Ohio   43062.  Speed Commerce, formerly Speed FC, Inc.,  was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard (29CFR 1904.29(b)(1)), which was contained in OSHA inspection number 109769, citation number 2, item number 1 a and was affirmed as a final order on January 18, 2012, with respect to a workplace located at 107 Heritage Drive, Pataskala, Ohio   43062.
