29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents. (a)ICM Services LLC @ 3400 Valentia Street, Denver, CO 80238: On and before December 11th, 2014, the exposing employer failed to provide his employees with adequate eye protection. Specifically, two employees were observed utilized a pneumatic nail gun to attach roof shingles without eye protection. The condition exposed employees to the hazard of projectile to the eye.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure. (a)ICM Services LLC @ 3400 Valentia St., Denver, CO 80238: On and before December 11th, 2014, the exposing employer failed to ensure that his employees were provided with adequate fall protective measures while engaged in roofing work. Specifically, employees were observed performing roofing work on an approximately 20 foot, 5 in 12 pitched roof, two story single family residential home without fall protective measures in place. The condition exposed employees to fall hazards of approximately 20 feet.
