29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level:  Affinity Project: On or about December 11, 2014 and at times prior, a worker on a roof with an eave height of 36 feet rigged his rope-grab lanyard such that the worker would fall approximately 20 feet before the fall was arrested.  Abatement Note: Abatement certification is required for this item (see enclosed Certification of Corrective Action Worksheet).

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a) Establishment Records: On or about December 11, 2014 and at times prior, The employer did not have a record of training for each of their employees exposed to fall hazards.  Abatement Note: Abatement certification and documentation are required for this item (see enclosed Certification of Corrective Action Worksheet).  Acceptable documentation consists of a certificate of training for each employee potentially exposed to fall hazards.
