29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s). (a) The employer did not conduct inspections of the work site to ensure employees conducting roofing work were not exposed to hazards. This allowed for employees to work on the roof of a commercial building without fall protection, exposing them to a fall hazard of approximately 16.3 feet.

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. (a) The employer failed to ensure that each employee conducting roofing work at W226N880 Eastmound Drive in Waukesha, Wisconsin, were protected from falling to the lower level by the use of a fall protection system. This exposed employees to a fall hazard of approximately 16.3 feet. All City Contacting Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.501(b)(13)), which was contained in OSHA inspection number 313529216, citation number 1, item number 3, and was affirmed as a final order on December 3, 2010, with respect to a workplace located at 1916 & 1918 North 49th Street in Milwaukee, Wisconsin.
