(a) The employer failed to ensure that each employee conducting roofing work at W226N880 Eastmound Drive in Waukesha, Wisconsin, were protected from falling to the lower level by the use of a fall protection system.  This exposed employees to a fall hazard of approximately 16.3 feet.   All City Contacting Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard (29 CFR 1926.501(b)(13)), which was contained in OSHA inspection number 313529216, citation number 1, item number 3, and was affirmed as a final order on December 3, 2010, with respect to a workplace located at 1916 & 1918 North 49th Street in Milwaukee, Wisconsin.
