29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: On or about December 9, 2014 and prior thereto, in the north west side of the old tunnel, the end of the catwalk was not equipped with a guard rail exposing the employees walking and working in the area to the hazard of falling approximately 10 feet to the concrete floor below.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about December 9, 2014 and prior thereto, conveyors and rice processing equipment were not guarded, exposing employees processing rice and performing housekeeping duties to the hazard of being caught in the machines in the following instances: a) The Flexoveyor conveyor serial number 414E4790B b) The Flexoveyor conveyor serial number 1202F3086B c) The south end of the drag conveyor on the east side of the bin deck.
