OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to high pressure hazards:  At this oilfield waste disposal site, steel piping associated with a diesel-powered skid pump and its pressure release device was not anchored or adequately secured.  Employees were exposed to being struck by equipment in the event of sudden release of pressurized fluid.

29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered    At this oil field waste water disposal facility, at the unloading area only, the employer failed to provide and ensure the use of flame-retardant treated clothing (FRC) necessary to protect employees from burns due to potential flash fires.    Abatement:  Employer agrees to provide and insure use of flame-retardant clothing for personnel engaged in unloading operations.  Use of flame-retardant clothing is not required for personnel performing duties other than working in the loading area.

29 CFR 1910.145(c)(2)(i): Caution signs were not used to warn against potential hazards or to caution against unsafe practices:    At this oil field waste disposal site, signage was not posted to warn employees on or near the skid on which the diesel powered pump was mounted on to warn employees of the following hazards:    A. A sign indicating the diesel pump was out of service was not removed and replaced with a sign to warn of high pressure on the skid.    B. The throttle or clutch device was not identified as to its purpose to alert employees that movement of the throttle could increase the pressure in the flow line.
