You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year. a) At the establishment, column "F" of the OSHA-300, Log of Work-Related Injuries and Illnesses for 2014" did not include all the information required for the injury that occurred on August 28, 2014. Column "F" must include a description of the injury, the parts of the body affected, and the object/substance that directly injured or made the employee ill.
