29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents.    On or about December 10, 2014 and times prior thereto, employees of Arkansas United Contractors, LLC performed roofing activities on the rooftop of the facility being constructed at or near 13621 Highway 367 N. in Tuckerman, Arkansas.  Employees operated pneumatic nail guns without the use of eye protection.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:    On or about December 10, 2014 and times prior thereto, employees of Arkansas United Contractors performed roofing on an approximate 5:12 and 9:12 pitch rooftop located at or near 13621 Highway 367 N. in Tuckerman, Arkansas.  The employees were not protected from falling to lower levels with the use of guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about December 10, 2014 and times prior thereto, employees of Arkansas United Contractors, LLC performed framing and decking on the rooftop at the worksite located at or near 13621 Highway 367 N. in Tuckerman, Arkansas.  The employer had not provided a training program for each employee exposed to fall hazards.
