29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, a personal fall arrest system, or an alternative fall protection measure as provided by another provision in paragraph (b) of this section: a. On or about December 12, 2014, on the 6:12 pitch roof, employees installing OSB board roof sheathing were exposed to 9 feet to 17 feet fall hazards in that they were not protected by a fall protection system.

29 CFR 1926.503(a)(1): The employer did not train each employee who might be exposed to fall hazards to enable them to recognize fall hazards and the procedures to be followed in order to minimize these hazards: a. On or about December 12, 2014, on the 6:12 pitch roof, employees exposed to fall hazards had not been trained on lanyard length adjustments, anchor point installations, and the selection of fall protection anchor point locations in order to minimize fall hazards.
