29 CFR 1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:     a) 57 Armstrong Drive, Clark, NJ 07066. The employer allowed his employee to operate a Hitachi C 8FB2, 8-1/2 Slide Compound Saw to cut siding and trim while not wearing any eye and or face protection exposing the employee to eye and or face injuries from flying debris.       Condition occurred on or about 12/15/14.

29 CFR 1926.451(g)(4)(i): Guardrail systems were not installed along all open sides and ends of walking/working platforms:   a) 57 Armstrong Drive, Clark, NJ 07066. The employer allowed his employee to work on and from Alum-A-Pole pump jack scaffold with no fall protection systems in place, i.e. guardrails/netting installed along all open sides and ends of walking/working platforms exposing the employee to fall hazards.       Condition occurred on or about 12/15/14.

29 CFR 1926.1053(b)(4): Ladders were used for purposes other than the purposes for which they were designed:    a) 57 Armstrong Drive, Clark, NJ 07066. The employer allowed his employee to use the fly section of a Werner 36 foot aluminum ladder to access the roof when the ladder was not being used as intended by the manufacturer. The fly section does not have the appropriate safety shoes to prevent slippage exposing the employee to fall hazards.     Condition occurred on or about 12/15/14.
