29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. On or about December 15, 2014, at the job site located at 6702 Detroit Ave, Cleveland, Ohio 44102 during the installation of roofing material on a low slope roof, the employer did not ensure employees were protected from a fall of approximately 42 feet by utilizing a guardrail system, fall protection system/device, and/or safety monitor system. USA ROOFING, INC WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.501(B)(11), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 942052 , CITATION NUMBER 01, ITEM NUMBER 001, AND WAS AFFIRMED AS A FINAL ORDER ON MAY 21, 2014, WITH RESPECT TO A WORK PLACE LOCATED AT 1 LAWRENCE RD, PORT CLINTON OHIO 43452.
