Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of being struck-by fragments of Polyvinyl Chloride (PVC) plastic pipe in the event that the PVC plastic pipe ruptured:        Room containing the Burn Table:  On or about November 25, 2014, employees were exposed to the hazards of being struck by fragments of Polyvinyl Chloride (PVC) plastic pipe, while the pipe was being used to transport compressed air, at approximately 118 psi, to tools and equipment, while the pipe was not in a conduit or casing of sufficient strength to provide protection from external damage and deterioration.

29 CFR 1910.334(a)(2)(i): Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket). Cord and plug connected equipment and flexible cord sets (extension cords) which remain connected once they are put in place and are not exposed to damage need not be visually inspected until they are relocated:     Main Production Floor:  On or about November 25, 2014, employees were exposed to an electrical hazard while using a Maxx Air, 30 inch, pedestal fan, which was plugged into an energized circuit and the grounding pin of the plug was missing.
