29 CFR 1926.502(b)(1): The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level:Note: When employees are using stilts, the top edge height of the top rail, or equivalent member, shall be increased an amount equal to the height of the stilts. a) 2051 Bevin Dr. Allentown, Pa., 1st Level, Elevator Chute -- On or about December 12, 2014, employees were installing HVAC ducts and running wire throughout a two-story commercial facility, while being exposed to an inadequately guardrailed elevator chute which was approximately 14 feet above a lower level.

29 CFR 1926.502(b)(13): When guardrail systems were used around holes which were used as access (such as ladderways), they were not provided with a gate, or were not so offset that a person could not walk directly into the hole: a) 2051 Bevin Dr. Allentown, Pa., Mezzanine Area -- On or about December 12, 2014, employees were installing Heating, Ventilation, and Air Conditioning ducts and electrical wiring up in the mezzanine area, while being exposed to an unguarded floor opening which was approximately 10 feet above a lower level.
