29 CFR 1926.502(b)(1): The top edge height of the top rails, or equivalent guardrail system members, was not 42 inches (1.1 m) plus or minus 3 inches (8 cm) above the walking/working level: a) 2051 Bevin Dr. Allentown, Pa., 1st Level, Elevator shaft - On or about December 12, 2014, employees were performing electrical installation throughout a two-story commercial structure, while being exposed to an inadequately guarded elevator shaft opening, in that the top rail height was approximately 30 inches above the floor height, which created a fall hazard from 14 feet above a lower level.
