29 CFR 1910.178(q)(1): Any power-operated industrial truck not in safe operating condition was not taken out of service, and/or repairs were not made by authorized personnel:    In the following instances, power-operator industrial trucks not in safe operating condition were not taken out of service at the Washington Avenue facility:    a)    Yale forklift #1 - The left rear light was not working and was detached from the structure.      b)    Yale forklift #2 - The right front light did not work.  On or about the week of 12/1/2014, the back rest was not properly maintained and fell off the operator drove the forks into a stack.    c)    Yale forklift #4 - The left rear light was not working and was detached from the structure.  The left front light was not working.  The emergency brakes were on the "soft side".    d)    Yale forklift #6 - The emergency brake did not work.    e)    Yale forklift #7 - The right rear light was not working.

29 CFR 1910.303(b)(2): Listed or labeled equipment were not installed and used in accordance with any instructions included in the listing or labeling.   In the following instances, portable powered tools were not used according to its listing or labeling:    a)  Repair Station #11 - The plug attached to the double insulated Bosch reciprocating saw was modified to a 120 volt three wire twist lock plug.  In addition, the rubber boot was missing, exposing the metal frame which had 26 volts to ground.   b)  Repair Station #12 - The plug attached to the double insulated Bosch reciprocating saw was modified to a 120 volt three wire twist lock plug.  In addition, the rubber boot separated and exposed a portion of the metal frame which had 28 volts to ground.   c)  Repair Station #3 -  The plug attached to each double insulated SENCO screwdriver gun was modified to a 120 volt three wire twist lock plug.  In addition, the gun's metal feed housing had 29 volts to ground.    d)  Repair Station #5 - The plug attached to each double insulated SENCO screwdriver gun was modified to a 120 volt three wire twist lock plug.  In addition, the gun's metal feed housing had 12-25 volts to ground.    e)  Repair Station #6 - The plug attached to each double insulated SENCO screwdriver gun was modified to a 120 volt three wire twist lock plug.  In addition, the gun's metal feed housing had 30 volts to ground.    f)   Repair Station #8 - The plug attached to each double insulated SENCO screwdriver gun was modified to a 120 volt three wire twist lock plug.  In addition, the gun's metal feed housing had 4 volts to ground.    g)  Repair Station #9 - The plug attached to each double insulated SENCO screwdriver gun was modified to a 120 volt three wire twist lock plug.  In addition, the 2 week old gun's metal feed housing had 9 volts to ground.
