OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment in a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the unregulated release of anhydrous ammonia from the refrigeration system resulting in employee exposures to chemical burns and inhalation hazards from: a. Not having an inspection and preventative maintenance program for the system (IIAR) Bulletin 110 Section 6 b. Not properly identifying and labeling the piping systems (ASME A13.1-2007) c. No functional ammonia vapor detection system ( ANSI/ASHRAE-15 (2010) Section 8.11.2.1 d. The mechanical ventilation fans were not functioning (ANSI/ASHRAE-15 (2010) Section 8.11.3 e. There was no remote control of the mechanical equipment within the machinery room ANSI/ASHRAE-15 (2010) Section 8.12(I) f.  The signs and warnings outside the mechanical rooms were not in accordance with ANSI/IIAR 2-2008 Section 13.1.10.4.
