29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards. On or about December 11, 2014, and at times prior thereto, employees were working on top of a flat roof that was more than four feet above ground without using standard guardrails (or equivalent) and toe boards. This condition exposed employees to fall from heights greater than for feet and strike-by hazards from falling objects.

29 CFR 1910.25(d)(2)(xv): No ladder should be used to gain access to a roof unless the top of the ladder shall extend at least 3 feet above the point of support, at eave, gutter, or roofline. On or about December 11, 2014, and at times prior thereto, employees were using a ladder to access a flat roof of a commercial building to perform tree trimming activities that was not extended at least 3 feet above the point of support. This condition exposed employees to fall hazards from height greater than four feet.
