29 CFR 1915.73(d): When employees were exposed to unguarded edges of decks, platforms, flats, or similar flat surfaces, more than 5 feet above a solid surface, the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1) and (2): a) 1818 Brown Ave., Norfolk, VA 23504, Vessel APL-50: On or about December 16, 2014, and times prior thereto, the vessel was not equipped with a complete and adequate guardrail system, thus exposing employee(s) to fall hazards and injuries such as contusions, broken bones, lacerations, drowning, and death. No abatement certification or documentation required.
