29 CFR 1910.268(j)(3): Vehicle-mounted elevating and rotating work platforms shall not be operated with any conductive part of the equipment closer to exposed energized power lines than the clearances set forth in Table R-2 of this section: a) Cornell Rd., Laceyville, PA: The employer did not ensure that a vehicle mounted elevating and rotating work platform was not operated closer to exposed energized power lines than 36 inches in that an employee was fatally injured when he contacted an overhead energized 19900 volt power transmission line while using a non-insulated aerial lift to install telecommunication lines, on or about December 15, 2014. Abatement Certification and Documentation Required. 29 CFR 1910.268(c): The employer did not provide training in the various precautions and safe practices described in (1) through (3) of this section and did not ensure that employees did not engage in the activities to which this section applies until such employees have received proper training in the various precautions and safe practices: a) Cornell Rd., Laceyville, PA: The employer did not provide employees with training in the recognition and avoidance of dangers relating to encounters with harmful substances, procedures to be followed in emergency situations or in first aid training, including instruction in artificial respiration, on or about December 15, 2014. Abatement Certification Required.
