29 CFR 1915.74(a)(1): The employer allowed employees to access vessels by way of gangways not safely secured. a. On or about October 13, 2014, on board motor vessel Liberty Island, moored to a pier with a gangway system installed for employee access between the pier and the vessel, fell as it was not safely secured and resulted in an employee sustaining a strain type ankle injury.
