29 CFR 1910.212(b): Machine(s) designed for fixed location(s) were not securely anchored to prevent walking or moving:   On or about December 16, 2014, and at times prior thereto, employees were exposed to struck-by hazards while punching holes on approximately 4 feet X 6 inch metal stock, on unanchored such as, but not limited to:   a) Along the floor production area bay #1 a Mubea Ironworker punch press model KBL-50-4, serial number 131A/4435F/27.  b) Along the floor production area bay #1 a Mubea Ironworker punch press model KBL 1100, serial number 05060-01-676-648.

29 CFR 1910.253(b)(2)(ii): Assigned storage spaces for cylinders was not located away from elevators, stairs, or gangways where cylinders could not be knocked over or damaged by passing or falling objects, or subject to tampering by unauthorized persons:  On or about December 16, 2014, and at times prior thereto, along bay # 1 employees were exposed to the hazards of struck-by while working around approximately 20 compressed gas cylinders containing chemicals such as oxygen and acetylene that were not secured and stored.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:  Along the production floor area, on or about December 16, 2014, and at times prior thereto, employees were exposed to electrical shock hazards of 120 and 220 volts from unused openings and breakers along the bottom and sides of the electrical distribution panel boxes that were not effectively closed:  a) Along the center of the production floor bay #3 enclosure panel E controlling welding machines.  b) Along the center of the production floor bay #3 Square D enclosure cat. no. Q0C16U controlling grinder plugs.  c) Along the center of the production floor bay #5 Square D enclosure cat. no. Q0C16U controlling welding machines.  d) Along the north entrance of the production floor bay #2 Panel H controlling the overhead door, gantry cranes, welding machines, drill press machines and electrical saws.  e) Along the north entrance of the production floor bay #2 Square D enclosure model A-40274-384-01 controlling the air compressor.
