29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: On or about December 10, 2014, procedures were not documented and utilized for the control of potentially hazardous energy when a pin hole leak in a weld was ground out and repaired on piping on the ammonia refrigeration system at HTX-18 Air Unit and Control Bank. Employees were exposed to hazards associated with the unexpected energization of the line with ammonia.
