29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) On a commercial flat roof in Brunswick, GA: On December 15, 2014, the employer exposed employees to fall hazards of 13 ft. 6 in. to the ground below, in that a 32-ft. portable ladder was extended approximately 18 in. above the upper-landing surface as employees accessed and egressed via the ladder.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work areas at the time of their initial assignment and whenever a new hazard that the employees had not been trained about was introduced into their work area. Employees were not trained regarding the new label elements and safety data sheets format by December 1, 2013 (Construction reference 1926.59): a) On a commercial flat roof in Brunswick, GA: On December 15, 2014, and times prior thereto, the employer exposed employees to chemical hazards of a weld-bonding adhesive and a weather-membrane cleaner in that employees were not provided information and training on the hazardous chemicals prior to performing roof-leak repair activities, nor were they trained regarding the new label elements and safety data sheets format required by the Hazcom 2012 update.
