29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  (a) Throughout Facility - On or about December 27, 2014, employee engaged in servicing and maintaining equipment with hazardous energy sources where specific procedures were not developed for the control of hazardous energy.  Abatement Note: The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following: 1.  A specific statement of the intended use of the procedure; Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy; 2.  Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and 3.  Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy control measures.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees:  (a) Throughout Facility - On or about December 17, 2014 each authorized employee did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.   (b) Throughout Facility - On or about December 27, 2014, where a servicing and maintenance was conducted on machinery and equipment with hazardous energy sources, each affected employee was not instructed in the purpose and use of the energy control procedure.   (C) Throughout Facility - On or about December 27, 2014, all other employees whose work operations are or may be in an area where energy control procedures may be utilized, were not instructed about the procedure, and about the prohibition relating to attempts to restart or reenergize machines or equipment which are locked out or tagged out.

(b) Plant 4 - On or about December 17, 2014, at the IWER Weaver Machine (#IW-7), a portable radiant heater unit is is wired directly into a fuse box with a Type SOO flexible cord, Super-Trex Ultra-Gard Portable Cord.
