29 CFR 1926.405(g)(1)(ii): Flexible cord was not equipped with an attachment plug and energized from a receptacle outlet when used as permitted in 29 CFR 1926.405(g)(1)(i) [C], [F], or [H]: On or about December 16, 2014, and at times prior thereto, the orange 25 foot extension cord had the plug end cut off and was hard wired into the 110 volt job made electrical distribution panel, exposing employees to the hazard of electrical shock.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws: On or about December 16, 2014, and at times prior thereto, strain relief devices were not provided on the following pieces of electrical equipment creating an electrical and fire hazard at the construction site: a. The 220 volt job made electrical distribution panel TB #1, located in the parking garage next to the base of the tower crane. b.The 110 volt job made electrical distribution panel the orange extension cord hard wired into the box located on the second floor. c. The 110 volt 25 foot orange extension cord plugged into the light site located on the 3rd floor.
