29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: On or about December 17, 2014, an employee was staging material on a framed commercial structure while not using any type of fall protection and exposed to a fall height approximately 13'-8".

29 CFR 1926.602(c)(1)(viii)(A): The industrial truck was not equipped with a safety platform/cage firmly secured to the lifting carriage and/or forks when used for lifting personnel: Located along the front area of the commercial structure, an employee was observed standing on the forks of a GEHL RS8-42 Telescopic Handler (rough terrain forklift) being lowered to the ground while the forklift was traveling in reverse. The employee was not protected by secured guard railed safety platform or equivalent and was exposed to a potential fall hazard in excess of 13 feet to the ground below.
