29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:      a) Advantage Metal Powders, Inc. / 44 Spleen Road, Ridgeway, PA: On or about December 17, 2014 and times prior thereto, the deck used by employees to load materials into the 8,000 lb blender was not equipped with a guard rail system that included a mid-rail and top rail. The deck was approximately 8 feet from ground level.    Or In The Alternative:    29 CFR 1910.132(a): protective equipment, in the form of personal fall-arrest systems, was not used wherever it was necessary by reason of hazards which were encountered in a manner capable of causing injury or impairment in the function of any part of the body:    a) Advantage Metal Powders, Inc. / 44 Spleen Road, Ridgeway, PA: On or about December 17, 2014 and times prior thereto, employees working on the deck to load materials in the 8,000 pound blender were not protected from falling to the ground by using a personal fall arrest systems (PFAS). Employees working on the deck area were exposed to a fall hazard of approximately 8 feet.

The employer was unable to provide a written certification that a workplace personal protective equipment hazard assessment had been completed.

The forklift data plate was not updated with the approximate weight of the truck and lift basket combination at maximum elevation with load laterally centered.
