B) The scaffold system that rested on the roof of the north porch was not fully planked and employees were exposed to a fall of approximately 15'-6", on or about 12/17/2014.  C)  The scaffold system facing the west side of the building was not fully planked and employees were exposed to a fall of approximately 14'-8", on or about 12/17/2014.
