Eastside of Home - On or about December 16, 2014, there was an employee working from a roof truss while trimming the corner of a 2x6 piece of lumber without any fall protection system in place, thus exposing the employee to a fall hazard of 19.45 feet onto earthen ground.   b). 2nd Floor Southwest Window - On or about December 16, 2014, employees were accessing an extension ladder through a window opening without any fall protection system in place, thus exposing employees to a fall hazard of 12.5 feet onto earthen ground.
