I-240 Service Rd, Oklahoma City, Insignia Signs, Inc did not ensure each employee was wearing a safety harness with lanyard attached to the proper anchor points while in the Wilkie aerial ladder basket exposing employees to a fall from approximately 20 ft to the ground below.
