29 CFR 1910.1048(m)(5): The employer did not develop, implement, and maintain at the workplace, a written hazard communication program for formaldehyde exposures in the workplace: Panolam Industries International, Inc. - on or about March 5, 2015, where employees were exposed to respiratory, eye, and skin, hazards due to formaldehyde used in the Hymmen and Siemplekamp presses, the employer did not develop and implement a Hazard Communication program for formaldehyde. An employee was exposed to formaldehyde at 0.1176 parts per million (ppm) as an 8-hour time-weighted average (TWA). Sampling was performed for 474 minutes with zero exposure assumed for the unsampled period of time (6 minutes), and to formaldehyde at 0.5700 ppm as a 15-minute time-weighted average with sampling time of 15 minutes. An employee was exposed to formaldehyde at 0.0932 parts per million (ppm) as an 8-hour time-weighted average (TWA). Sampling was performed for 472 minutes with zero exposure assumed for the unsampled period of time (8 minutes), and to formaldehyde at 0.6800 ppm as a 15-minute time-weighted average with sampling time of 15 minutes. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

An employee was exposed to formaldehyde at 0.1176 parts per million (ppm) as an 8-hour time-weighted average (TWA). Sampling was performed for 474 minutes with zero exposure assumed for the unsampled period of time (6 minutes), and to formaldehyde at 0.5700 ppm as a 15-minute time-weighted average with sampling time of 15 minutes.
