29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:    a) Highlands Square Subdivision, Lot 376 - On or about December 17, 2014 employees were not wearing eye protection while using  pneumatic nail guns to install roof felt paper, exposing employees to possible eye injuries from being struck by a nail.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) Highlands Square Subdivision, Lot 376 - On or about December 17, 2014, employees installing roof felt paper were exposed to fall hazards up to approximately 18.5 feet onto earthen ground because the workers were not fall protected.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a rigid support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the ladder:    a)  Highlands Square Subdivision, Lot 376 - On or about December 17, 2014, employees used an aluminum extension ladder to gain access to a second story balcony on the house without the side rails of the ladder being extended at least 3 feet above the upper landing surface nor was the ladder secured at its top to a rigid support, exposing employees to fall hazards of up to approximately 10.5 feet onto earthen ground.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) Highlands Square Subdivision, Lot 376 - On or about December 17, 2014, and at times prior to, the employer did not ensure employees engaged in residential roofing work and were regularly exposed to fall hazards up to approximately 18.5 feet, were effectively trained to recognize the fall hazards encountered in their work.  The employer lacked the knowledge to provide his employees with effective fall hazard recognition training.
