29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. An employee was exposed to a fall hazard while standing at the edge of a flat commercial roof providing hand signals to a crane operator without the use of any fall protection. Potential fall height was measured to be 16'-8".

29 CFR 1926.1428(a): The employer of the signal person did not ensure that each signal person met the Qualifications Requirements (paragraph (c) of this section) prior to giving any signals: An employee was observed standing at the edge of a commercial roof giving hand signals to a crane operator to hoist an Air Handler Unit to a roof top had not been trained by the employer meeting the qualifications requirements of a signal person.
