29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: The employer does not protect each employee from falls from roofs by an adequate fall protection system. Employees working from a 6 in 12 pitch roof are exposed to falls of approximately 10.2 feet to a lower level. Employees are not protected from falling by guardrail systems, safety net systems, or a personal fall arrest systems or any other approved means. L&J Construction Inc., was previously cited for a violation of this occupational safety and health standard or an equivalent standard contained in: OSHA Inspection number 911203, citation 2 item 2 issued on August 16, 2013, with respect to a workplace located at 301 W. St. Louis St., Lebanon, Illinois 62254; OSHA Inspection number 911069, citation 1 item 3 issued on July 22, 2013, with respect to a workplace located at 107 Center, Troy, Illinois 62294; OSHA Inspection number 872183, citation 1 item 3 issued on May 6, 2013, with respect to a workplace located at 716 Keebler Rd., Collinsville, Illinois 62234 OSHA Inspection number 783321, citation 1 item 2A issued on January 2, 2013, with respect to a workplace located at 5930 State HWY 162, Glen Carbon, Illinois 62234 Pursuant to 29 CFR 1903.19, the employer must submit documents describing the steps it is taking to ensure compliance, including an explanation of how these steps protect the employees from fall hazards.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. The employer did not ensure portable ladders were placed so three feet or more was extended above the upper landing. Employees preforming residential roofing were exposed to falling approximately 10.2 feet while using an extension ladder to ascend and descend from the roof. L&J Construction Inc. dba Oasis Construction Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053(b)(1), which was contained in OSHA inspection number 872183, citation number 1, item number 4a and was affirmed as a final order on June 3, 2013, with respect to a workplace located at 716 Keebler Rd., Collinsville, Illinois 62234
