29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides, a) Exterior-yard: Employees were standing on a tank without using any fall protection. An employee fell approximately 13'6" while engaged in rigging; on or about 12/1/14. b) Exterior-yard: An employee was exposed to a fall of greater than 6 feet while working in a man basket without a fall protection system; on or about 12/3/14.

Category 1 or 2 flammable liquids, or Category 3 flammable liquids with a flashpoint below 100 °F (37.8 °C), shall not be handled, drawn, or dispensed where flammable vapors may reach a source of ignition. Smoking shall be prohibited except in designated localities. "No Smoking" signs shall be conspicuously posted where hazard from flammable liquid vapors is normally present. a) Building interior- warehouse: Employee was smoking while standing next to an employee using a shrink wrapping machine fueled by a propane tank; on or about 12/15/14.
