29 CFR 1904.39(a): The employer did not orally report the death of an employee or the in-patient hospitalization of three or more employees because of a work-related incident: a) Establishment: Macys Store 72, Rockaway Townsquare Mall, Rockaway, NJ 07866 and Corporate Office On or about 12/16/14, an employee suffered a heart attack while working at the Macys Rockaway Townsquare facility in Rockaway, NJ. The employer failed to report the death of the employee to the Occupational Safety and Health Administration (OSHA) within eight hours of the incident. Violation disclosed on or about 12/18/2014.
