29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: At the facility, 2604 Industrial Road, Atchison, Kansas, an employee was changing an ear punch die on the Anchor forging press #3 and did not lockout the energy sources for the machine. Employee was exposed to an amputation hazard.

29 CFR 1910.218(a)(3)(iv): The ram shall be blocked when dies are being changed or other work is being done on the hammer. Blocks or wedges shall be made of material the strength and construction of which should meet or exceed the specifications and dimensions shown in Table O-11. At the facility, 2604 Industrial Road, Atchison, Kansas, an employee was changing a die in Forging Press #3 and a ram block was not used. Employee was exposed to amputation hazard.
