29 CFR 1910.22(a)(1): All places of employment, passageways, storerooms or service rooms were not kept clean and orderly or in a sanitary condition.    On or about December 19, 2014, the workplace was not kept in a clean, orderly, and sanitary condition. A fire at the facility was exacerbated by the oil-slicked dust coating the walls, floors, rafters, and equipment in the facility.

29 CFR 1910.95(g)(1): The employer did not establish and maintain an audiometric testing program as provided by 29 CFR 1910.95(g) by making audiometric testing available to all employees whose exposures equal or exceed an 8-hour time-weighted average of 85 decibels:    On or about January 14, 2015, the employer failed to conduct audiometric testing for employees whose noise exposure was at or above the action level, of 85 decibels, for an 8-hour time weighted average.
