All personal protective equipment did not meet the requirements nor was it maintained according to subparts D and E of this part:    Jobsite:  Employees were performing roofing operations on a roof with two different levels using pneumatic nail guns without the use of eye protection exposing employees to being struck-by falling or flying objects.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    Jobsite:  Three employees were engaged in roofing activities on a low-slope roof without the use of guardrails, safety nets, warning lines, personal fall arrest systems, or safety monitor exposing them to a fall of approximately 10 feet to the pavement below.
