29 CFR 1926.502(d)(11): Lifelines were not protected against being cut or abraded:    a) On or about December 22, 2014 - job site at 85-11 34th Ave, Jackson Heights, NY 11373    The employer permitted an employee to work on a two point suspended scaffold while wearing a harness, attached to    a slacked lifeline, that was covered with construction debris.  This debris consisted of broken bricks and lumber.    WRITTEN ABATEMENT CERTIFICATION IS REQUIRED.

The slack in the lifeline would permit the employee to free   fall more than 6 feet.  WRITTEN ABATEMENT CERTIFICATION IS REQUIRED.
