29 CFR 1910.134(e)(1): The employer did not provide a medical evaluation to determine the employee's ability to use a respirator, before the employee was fit tested or required to use the respirator in the workplace: (a) Fabrication Department/Bay 8: An employee was wearing a Miller brand tight fitting air purifying respirator while welding and the employee had not been medically evaluated prior to wearing the respirator as determined on December 22, 2014. ABATEMENT CERTIFICATION REQUIRED The Conn-Weld Industries, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.134(e)(1), which was contained in OSHA inspection number 315224626, citation number 1, item number 3a and was affirmed as a final order on September 28, 2011, with respect to a workplace located at 315 Wabash Road, Princeton, WV.

29 CFR 1910.1026(j)(1)(i): The employer did not ensure that all surfaces were maintained as free as practicable of accumulations of chromium (VI): (a) Fabrication Department: Two tool boxes used by employees in the area where welding operations were conducted were found to have contamination of hexavalent chromium on them as determined on January 9, 2015. ABATEMENT CERTIFICATION REQUIRED
