29 CFR 1926.652(a)(1): Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section:     a) On or about December 22, 2014, Pridemore Construction Inc. did not ensure that each employee in an excavation was protected from cave-in hazards.  Employer did not use an adequate protective system in accordance with OSHA 1926.652(c) standards. This condition exposed the employee to a crushing hazard by trench wall cave-in. 29 CFR 1926.651(k)(2): Where the competent person found evidence of a situation that could result in a possible cave-in, indications of failure of protective systems, hazardous atmospheres, or other hazardous conditions, exposed employees were not removed from the hazardous area until the necessary precautions have been taken to ensure their safety.   (a) On or about December 22, 2014, the employer as the competent person, while installing a sewer line did not protect employees from cave-in hazards.  The employer did not identify hazardous conditions and remove employees from the excavation site.
