29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  a) On or about 22 December 2014, the employer did not use electrical space heating equipment in accordance with the manufacturers instructions.  In the Safety and Accounting Departments and in the hallway connecting these offices, flexible extension cords were plugged into other extension cords and/or relocatable power taps in order to extend their length.   This condition is prohibited by the manufacturer.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points:  a) On or about 22 December 2014, the employer did not ensure that flexible cords and cables were protected against accidental abrasion damage caused by running them through doorways.   Flexible extension cords cable extending between the hallway and the Safety and Accounting Departments were run through the doorway.  The cables were not protected against abrasion by the door edge.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
