29 CFR 1910.124(g)(2): Employees who  worked with liquids that could burn, irritate, or otherwise harm their skin were not provided with an emergency shower and eye-wash station close to the dipping and coating operation:  a.  On or about 12/22/14, employees working in the dipping area were exposed to material that could cause irritation to the eyes and skin and were not provided the required safety shower/eye wash.  Note:  In place of this equipment,  the employee may use a water hose that is at least 4 feet (1.22 m) long and at least 3/4 of an inch (18 mm) thick with a quick-opening valve and carrying a pressure of 25 pounds per square inch (1.62 k/cm2) or less.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  a. On or about 12/22/14, in the wood shop, employees were exposed to rotation of the drill and chuck on a Delta Drill Press (model # 17-900, serial # 9232) without any method of approved guarding.

Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling.  a)  On or about 12/22/14, in the wood shop, the 4" gang boxes were not listed and labeled to be used as pendant electrical drops.  b)  On or about 12/22/14, in the wood shop, there was a household re-locatable power tap powering the Delta Drill Press that was not used in accordance with the listing and labeling instructions.
