29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:    a) On or about 22 December 2014, the employer did not use electrical space heating equipment in accordance with the manufacturers instructions:      Instance # 1  Space Heaters including but not limited to Twinstar Model 10QI081ARA Heater; Lasko Model 5141 Ceramic Tower Heater; and LifePro Model LS-IRTWR-1 Infrared Quartz Tower Heater were plugged into extension cords and/or reels.  This condition is prohibited by the manufacturer.      Instance # 2  Extension cords and/or relocatable power taps were plugged into other extension cords and/or relocatable power taps in order to extend their length.   This condition is prohibited by the manufacturer.      In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1910.305(a)(2)(x): Flexible cords and cables were not protected from accidental damage, as might be caused, for example, by sharp corners, projections, and doorways or other pinch points:    a) On or about 22 December 2014, the employer did not ensure that flexible cords and cables were protected against accidental abrasion damage caused by running them through doorways:      Instance # 1  In the bathroom area the cable from an extension cord reel, powering a Lasko Model 5141 Ceramic Tower Heater, was run through the doorway.  The cable was not protected against abrasion by the door edge.      Instance # 2  In the office area an extension cord, powering a LifePro Model LS-IRTWR-1 Infrared Quartz Tower Heater, was run through the doorway.  The cable was not protected against abrasion by the door edge.    In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

Instance # 2  In the storage area, an extension cord was run through a hole in a wall in order to connect into an electrical outlet.
