29 CFR 1926.952(a)(2): The employer did not ensure that the employee in charge conducts a job briefing that meets paragraph (b),(c) and (d) of this section with the employees involved before they start each job: On or about December 18, 2014 and in times prior thereto, in the Hidden Forest Subdivision of the Canyon Lake District, electrical utility workers were rigging up to pull URD cable through underground risers located below 14.4 KVA power lines without conducting a job safety briefing to identify hazards and control measures, exposing employees to the hazards of electric shock.

29 CFR 1926.959(d)(1): Mechanical equipment was not operated so that the minimum approach distance established by the employer was maintained: On or about December 18, 2014 and in times prior thereto, employees were setting up a digger derrick truck to pull underground cable and failed to maintain a minimum 2' 2" approach distance, exposing employees to the hazards of electrical shock of 14.4 KVA.

29 CFR 1926.959(d)(2): A designated employee other than the equipment operator did not observe the approach distance to exposed lines and equipment and provide timely warning before the minimum approach distance require by paragraph (d)(1) of the section was reached.: On or about December 18, 2014 and in times prior thereto, a spotter was not designated to observe the digger derrick truck as the operator moved a spool of cable into position under overhead energized power lines. A 2' 2" minimum approach distance was not maintained. This condition exposed employees to a electric shock hazard of 14.4 KVA, located at the new power line construction site, in the Hidden Forest Subdivision.
