29 CFR 1926.959(d)(2): A designated employee other than the equipment operator did not observe the approach distance to exposed lines and equipment and provide timely warning before the minimum approach distance require by paragraph (d)(1) of the section was reached.:  On or about December 18, 2014 and in times prior thereto, a spotter was not designated to observe the digger derrick truck as the operator moved a spool of cable into position under overhead energized power lines.
