29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and on an incident report (OSHA Form 301or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness had occurred:    (a)  On or about December 23, 2014 - Metalsa Tuscaloosa, Inc. site located at 1150 Industrial Drive, Tuscaloosa, AL,  recordable injuries that occurred to contractor labor employees  working were not recorded on the host employer's 2014 OSHA 300 Log of Injury and Illness.

29 CFR 1910.134(k)(6): The employer did not provide the basic advisory information on respirators, as presented in Appendix D of 29 CFR 1910.134, in written or oral format to employees who wear respirators when such use was not required by the employer:    (a)  On or about 12/23/14 - Elastocast spray booth, a 3M-8200 face filtering dust mask was provided where employees were applying Elastocast 55090R urethane system and cleaning spraying equipment with Chem Strip 5015.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:    (a)  On or about 12/23/14 -  Elastocast spray area, employees hired through the contract labor provider were not provided specific hazard communication information about the chemicals used in the work area at the time of their assignment.   Potential exposure to isocyanate in the spray elastomer system and corrosive alkaline cleaning chemicals should be included in the training.
